Shoulder joint arthroplasty

Shoulder arthroplasty is the surgical replacement of a diseased joint with an artificial implant. This operation is performed in case of inevitable destruction of the joint capsule structures, when medications, exercises and minimally invasive procedures are no longer able to improve the situation. An endoprosthesis is the best and only option to get rid of daily pain and restore the usual mobility of the arm to full amplitude.

INDICATIONS:

If you experience constant acute or background pain in the shoulder, your movements are limited, there is deformity or swelling, these are significant symptoms for arthroplasty.

General indications for joint replacement:

  • progressive osteoarthritis;
  • improperly fused fractures;
  • chronic polyarthritis;
  • displacement of the shoulder joint;
  • dysplasia of the shoulder joint;
  • scapula fracture with damage to the articular socket;
  • osteonecrosis;
  • necrosis of the humeral head;
  • infectious disease - omarthritis.

Prosthetics may be impossible or postponed for some time if the patient has any of the contraindications listed.

CONTRAINDICATIONS

  • Heart, liver, lung, or kidney failure.
  • Severe cardiovascular diseases.
  • Neurological paralysis.
  • Chronic osteoporosis.
  • Infectious diseases in the inflammatory stage.
  • Obesity of 3-4 degrees.
  • Bone necrosis.
  • Diabetes mellitus in the stage of decompensation.

TYPES OF PROSTHETICS

Joint injuries or diseases can vary, so prosthetics differ in the type of construction and the list of components that should be replaced with artificial ones.

  • Partial prosthetics. If the patient's rotator cuff is mobile and without damage, then only the cartilage is replaced, without removing bone tissue. Rehabilitation in this case is the fastest.
  • Unipolar. When only one joint element is to be replaced: the head of the shoulder or the articular surface of the scapula. Recovery after this operation is also easier.
  • Total prosthetics. It is used in case of severe joint damage. During the operation, all joint elements are replaced with a ready-made implant.
  • Reversal. If there is a tear of the rotator cuff and major destruction of the joint, the elements of the endoprosthesis are reversed so that the polyethylene insert is attached to the humerus and the metal head is attached to the articular surface of the scapula.

HOW THE ARTHROPLASTY OPERATION IS PERFORMED

On average, the procedure lasts 1.5-2 hours. The patient is administered general anesthesia or local anesthesia, which blocks sensation in the nerve cells of the operated area. The surgeon makes a small incision in the shoulder area, removes damaged tissue and cartilage. A metal plate is attached to the shoulder joint, and then the joint prosthesis is screwed into the plate. It is fixed with cement, which connects the implant to the bone. Then the soft tissues are sutured in layers and a drainage tube is installed to drain excess fluid. An important factor for a quick recovery is proper rehabilitation after arthroplasty.

REHABILITATION AND RECOVERY

Immediately after the surgery, the arm is fixed with a special bandage or bandage in a bent position to protect the joint from mobility. The recovery period is accompanied by painkillers.
